The service operates 7 days a week with staff working on a rota basis and alternate weekends Experience of delivering frontline care including Reablement. Knowledge of internal policies and procedures relating to social care including knowledge of Care Quality Commission standards and regulations Knowledge of a range of presenting conditions –Understanding of related legislation. Manual handling practices and techniques & knowledge of completing risk assessments – and the ability to undertake reviews and complex risk assessments Developed IT skills including the use of Microsoft Office. Being an Assistant Reablement Coordinator involves supporting people in their own homes for a short space of time to assist them to reach their full potential, this may include managing a variety of aspects of daily living skills, set clear and achievable goals to help people rebuild their former independence and confidence to sustain independent living at home and in their communities. You will effectively work within the reablement team to deliver a high quality reablement service with the aim of promoting and sustaining people’s independence living in the community. complete service users journeys well which coordinated and complies with organisational policies, procedures and Care Quality Commission regulated guidance.
