Reports to
Care Manager
Summary of role
Responsible for implementing field-based quality control systems and providing support to all Care & Support Workers within a defined area, with the ability to work within a team environment.
Principal responsibilities
9. To carry out Individual Needs Assessments for new and potential clients.
10. To conduct risk assessments as part of the Individual Needs Assessment and update these as required as part of the individual’s risk management.
11. To ensure all client visits are always covered.
12. On-Call duty.
13. To maintain close liaison with both the Care Manager and Care Coordinator.
14. To identify suitable Care & Support Workers to cover for any absences, this may be due to sickness or holiday.
15. To facilitate the practical induction of all new Care & Support Workers through shadow shifts.
16. To provide and facilitate in-field training as and when appropriate.
17. To identify any additional training needs of Care & Support Workers and report accordingly.
18. To conduct client reviews and respond to Individual Review Forms completed by Care & Support Workers as soon as possible.
19. To urgently report any changes in a Care & Support Worker’s or client’s schedule to the Care Coordinator.
20. To monitor, supervise and mentor all Care & Support Workers in the field.
21. To provide support to all Care & Support Workers in the field.
22. To monitor all clients for service and Care & Support Worker satisfaction.
23. To ensure accuracy and completeness of all documentation held in clients’ homes.
24. To encourage and identify opportunities for Care & Support Workers continuing professional development.
25. To ensure that all written reports and administration is up to date.
Field Care Supervisors are responsible for the efficient running of their own specific, defined areas and should vary their hours of work to effectively monitor and support all Care & Support Workers and clients.
Requirements
26. U.K. driver’s license and vehicle.
27. Right to work in the U.K.
28. Experience of domiciliary Care, and a working knowledge of the care industry.
29. NVQ 2 or higher in Health & Social Care
30. Processing new referrals and changes to clients care and support plans.
31. Work with the office team to maintain up to date electronic/handwritten records.
32. Carry out client reviews on site.
33. To carry out spot checks and supervisions and assist the other Care office staff with aspects of their role.
34. Updating of care plans and providing support to the Care Teams.
35. On call duties
36. Able to work on your own and as part of a supportive team.
37. problem solving ability and a drive to succeed.
38. Previous experience with Access Care Planning desired, but not essential.
