Job Summary
Would you like to work for an innovation team? The Hospice at Home Team cares for patients in the last 2 - 4 weeks of life at home within Bournemouth and Christchurch. We are looking to expand our current Hospice at Home Service to include the Poole conurbation. The Hospice at Home service is a 24 hour/7 days a week service, based at the Macmillan Unit, Christchurch Hospital, predominantly visiting patients and families in their own homes to provide end of life care. Candidates will need to be able to drive and have a current UK licence, have experience in the care environment with the confidence to work independently and be flexible with working patterns to include nights. There will be an induction for any training requirements on starting.
Main Duties of the Job
1. To work independently and assist in carrying a caseload under the supervision of a senior nurse, performing tasks according to specialist nursing need/assessment at end-of-life.
2. To provide essential holistic, high-quality nursing care as directed by individualised care plans in the patient's own environment, respecting their backgrounds and beliefs.
3. To provide hands-on practical care and emotional support to patients and their families/carers, reporting any changes to the senior nurse.
4. To prepare the patient's environment for care at home, maintaining their dignity and privacy and ensuring a safe working environment for patients, families, staff, and colleagues within the Lone Worker Policy.
5. Recognise the Equality and Diversity of patients, relatives, and MDT groups, by supporting non-discriminatory practice & acknowledging the patient's personal culture, beliefs, and identity.

Skills
Technical Skills Competencies Essential:
* Ability to communicate clearly and effectively (to understand and be understood)
* IT Literate
* Ability to work as part of a team, as well as on own initiative
* Understanding of basic end of life symptoms and physiology
Desirable:
* Confidence in the use of EPR, CHIE, DCR
Qualifications
Essential:
* Current UK driving License
* Completion of competency-based Care certificate including workbook equivalent to level 3 or equivalent
* Good standard of Numeracy and Literacy
Desirable:
* Short Courses to undertake patient and clinical care duties in End of life care
Experience
Essential:
* Minimum of 1 year experience of working as a Health Care Support Worker in End of Life Care within a hospital or community setting
* Understanding and recognising the need for personal resilience in Palliative and End of Life Care
Desirable:
* Previous experience of working within a care environment/NHS
* Experience of working within patients' homes
* Recognising and recording changes in the dying patient, preparing their family members and carers in a time-sensitive and supportive manner
Knowledge
Essential:
* An understanding of and experience in end of life care

* Knowledge of care and related procedures, clinical observations, relevant legislation particularly in end of life care
