About this opportunity
Reablement Support Workers will be responsible for working with people identified with reablement needs to ensure key accountabilities. Driving licence is essential.
Job Accountabilities:
* Reablement goals/outcomes are met in accordance with the reablement plan.
* Any additional goals/outcomes are identified which may improve quality of life and promote independence during the reablement episode.
* Feedback about additional identified goals/outcomes is communicated to the Reablement and Assessment Officer.
* Clear and timely communication with the Reablement and Assessment Officer regarding the individuals' identified goals/outcomes being met.
* Close team working within a small dedicated team of reablement support workers, communicating effectively about progress or concerns in achieving goals.
