The Northumberland Recovery Partnership has an opportunity for Band 6 Community Practitioners to work within the team.
The role entails working with those affected by substance and alcohol misuse providing safe and effective interventions, harm reduction advice, and use of psychosocial interventions to support service users to consider and achieve recovery.
Main Duties of the Job
Physical Health Nurse
The post holder will work within our physical health pathway within the multi-disciplinary team to provide care to those affected by substance use.
The role involves completing comprehensive physical health assessments in line with Trust standards and offering appropriate interventions, onward referrals, and monitoring of health.
Older Persons Nurse
The post holder will work within the new Older Person's pathway within the multi-disciplinary team to provide care to those affected by substance use.
Detailed Job Description and Main Responsibilities
To be responsible for the review and management of casework of service users who have substantial and complex needs.
Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies, and the public.
In collaboration with the Service User and Carer, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.
To deliver evidence-based Clinical and Psychologically based Interventions.
The post holder will work as a member of the Multi-disciplinary Team ensuring that high-quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing.
Person Specification
Education and Qualification
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
* Proven track record of post-registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
* Up-to-date knowledge/experience of relevant legislation.
Skills and Competencies
* Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
* Risk Assessment and Risk Management/Safety Planning skills.
* Demonstrate an understanding of effective & appropriate treatment options including a range of evidence-based clinical approaches.
* Demonstrate effective written and verbal communication skills.
Knowledge and Experience
* Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
* 12 to 24 month band 5 qualified nurse experience dependent upon service need.
You must have appropriate UK professional registration.
