An exciting opportunity has arisen for a Street Triage Practitioner to support the Street Triage Team based at Cheshire West.
We are looking for a motivated and resourceful individual to join our team, who possess commitment to team working and innovative delivery of excellent standards of work.
We are a supportive, competent and caring team. The Post holder will be expected to work as part of the Urgent Care Mental Health First Response Service which includes Crisis Triage and Response Services (Crisis Line/First Response), Liaison Psychiatry, Home Treatment Teams and Street Triageto improve the safety, health and well-being of the people within our area.
Main duties of the job
To provide comprehensive assessment for people experiencing mental health crisis in a community setting whilst working in partnership with police colleagues.
Specialising in the inter-relationship of physical illness, psychosocial disturbance, psychosomatic disorders as well as the care and management of patients with mental health problems.
To provide mental health and risk assessment to persons presenting with episodes of self-injury or self-poisoning
Implementation and advice provision on treatment and management plans in collaboration with multidisciplinary colleagues at all levels, service users and carers.
Provide training to staff and police colleagues about mental illness and treatment.
