What we ask from you? -Working in a multi-disciplinary team alongside Clinical and Operational Leadership to ensure high quality care delivery to service users. -Provide specialist assessment and care planning in conjunction with service users and carers. - Evaluate treatment plans and undertake discharge planning and promote self care. -Be responsible for an agreed caseload. -Co-ordinate the work of the Team. -Support development of skills and competencies in others.
