Community Nurse - Learning Disabilities - Band 6
An exciting opportunity has arisen for an experienced and highly motivated individual to join the community city-wide service within the Learning Disabilities Service.
You will be an experienced and flexible individual with excellent communication skills and a passion for multi-disciplinary team working.
Main duties of the job
1. To work within the framework of a multi-disciplinary team to provide support and guidance to children and adults with learning disabilities and their carers.
2. To work within the framework of health facilitation (level one and two).
3. To co-ordinate a clinical service under the supervision of Senior Community Nurses.

Job responsibilities
1. To manage and co-ordinate a caseload under the supervision of the Senior Community Nurse.
2. To focus on individual health outcomes for people with learning disabilities and offer support to access health services.
Person Specification
Qualifications/training
* Registered Nurse: Learning Disabilities or Mental Health
* ENB 998 or equivalent
Experience
* Working with Multi-disciplinary Teams
* Dealing with complex health needs
* Working with Multi-agencies
Skills
* Knowledge and understanding of assessment processes
* Level 1 & 2 health facilitation
* Understand legislation in relation to people with learning disabilities
* Ability to undertake and understand audit tools
* Knowledge of Care Programme Approach
* Good communication skills both written and verbal
* Confident
Other Job Requirements
* Car driver essential with full licence and access to a vehicle
* Must be able to meet the travel requirements of the role
* On call participation
