An exciting opportunity! The Flame Lily Healthcare is currently having requirement for Care coordinator experienced to supervise the care delivery within the service to start Immediately.
Expectations & Requirements:
* To provide leadership to care staff within the services
* To provide care in accordance with current best practises, according to policy and procedures, agreed standards, legislative requirements, and relevant regulations under the direction of the Manager.
* To maintain skills at a current level, and undertake such training and development as may from time-to-time be required to maintain practises as up-to-date.
Roles & Duties :
* This role will require travelling to our services across Kent and other parts of the UK
* Ensure Service Users are at the heart of the care delivery and their wishes and preferences enhance their well-being.
* To ensure that all staff contribute to the efficient running of the service and supervise staff.
* To organise and coordinate care delivery in accordance with current best practises, according to policy and procedures, agreed standards, legislative requirements, and relevant regulations under the direction of the Manager and within the financial plans.
* Ensure that the care service is delivered to Service Users at the times and for the duration specified in the relevant individual agreement(s) and contract standards.
* Be responsible for promoting and safeguarding the welfare of those individuals they support.
* Compliance checks (inventory, spot checks, staff supervisions)
* Management of service user finances (applying for benefits)
* Preparation of care plans, risk assessment for new service user.
* PPE orders for all services (Children and Adult both)
* Medication Audit (monthly)
* Delivery of PPE on all services
* Updating care plans and risk assessment for existing services after monthly review.
* Training on technology skills.
* Participation in New joiners’ induction
* Sign off new joiners.

* Develop effective working relationships with other employees within care services
* Work in cooperation with members of the multi-disciplinary teams to maximise opportunities for Service Users.

Ability to commute/relocate:
* The Head office is based in Kent but the role requires travelling to various services across UK. You must hold a driving licence for this role. The right candidate will also have the option to get insured on the company car.
Experience:
* providing care: 1 year (preferred)
* Work Location: Can be from service locations.
Average Weekly Core Hours: 8.45am – 6pm (7.5 hours) Monday to Friday
