As a Social Worker in the Looked After Through Care Team, you will manage a caseload of children and young people in care, providing them with consistent support, guidance, and planning to ensure they achieve positive outcomes and feel empowered as they move towards independence.
Responsibilities include:
• Managing a caseload of looked-after children and young people, conducting regular assessments and reviews.
• Developing and implementing through care plans tailored to each young person's needs, ensuring their safety, well-being, and personal development.
• Working closely with foster carers, family members, and professionals from various agencies to provide a holistic support network.
• Preparing reports and assessments for reviews, care planning meetings, and court proceedings.
• Offering advice, advocacy, and support to young people transitioning from care to independence, helping them access resources and services.
To be successful, you will require:
• A qualified Social Worker registered with Social Work England.
• Experience working with looked-after children, preferably in a through-care or leaving-care team.
• A strong understanding of child protection legislation, safeguarding procedures, and care planning processes.
