Job summary St Pauls Primary Care Network presents an exciting opportunity for an experienced Health Care Assistant to work across the four practices that form the Network, all housed in St Pauls Medical Centre in the heart of Cheltenham. The successful candidate will work across the four practices continuing to support the successful home visiting service, and in addition supporting the PCN and Practices with patient annual health checks. This role will be closely supported by the wider PCN team of Allied Health Professionals. We are offering a part time position of 24 hours a week. Main duties of the job Provide direct care personalised support to patients needing Long Term Condition and Health Checks particularly: Learning Disability,Serious Mental Illness,Diabetes,Hypertension,Asthma,Dementia, AF. Provide a range of clinical diagnostic skills in order to provide annual health checks, recording onto the patients record and providing onward care and information.
