Job Overview The post holder will work to develop improved diagnostic, referral and treatment pathways for ACS patients across the care continuum from primary to tertiary care, engaging with healthcare professionals across the network area. The post holder will ensure that the service offered would be convenient and accessible to patients. It will assist in ensuring that patients receive timely and streamlined care as well as contributing to reducing waiting times, waiting lists for treatment and inappropriate admissions to acute beds. Main duties of the job To provide clinical leadership for a defined group of patients. You will be responsible for assessing, planning, implementing and evaluating safe and high quality patient care.
