Job summary
As part of the LTC team this role will focus on long term conditions such as Diabetes, Respiratory, Cardiovascular Disease etc. The role will consist of providing coordination and navigation for patients,
and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals whilst ensuring adherence to patient confidentiality and data protection.

This role will be based at Shipley Medical Practice, however you would be expected to work at different locations within Affinity Care.

Job responsibilities
Supporting the Quality Outcomes Framework (QOF) to achieve and support patients with specific disease areas.
Consider other outstanding QoF performance targets ensuring proactive contact with patient to book and encourage attendance for those appointments eg cervical screening.
Assist in raising awareness of health and well-being of our patients and their carers.
Work sensitively with patients and or their carers to capture key information, making every contact count, providing accurate and appropriate information.
Be the patients advocate, see things from a patient perspective; identifying ways to improve our systems to enhance the patients experience, capturing such knowledge in a way that it can be shared with other care coordinators and inform other teams within Affinity Care.
Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.
Seek advice and support from the clinical team to discuss patient-related concerns ( abuse, domestic violence and support with mental health), referring the patient back to the GP or other suitable health professional if required.
Develop and manage rotas and appointments system within SystmOne by liasing with the patient service managers and clinicians at the appropriate sites.
Booking appointments with the appropriate person/clinic and an appropriate amount of time.
Be part of the LTC team telephone network, receiving and making telephone calls to patients to book their reviews.
Communicate with patients via letters, SMS texts or by our online platform Patchs.
Support colleagues in the LTC team, remotely at our other practices and at site.

5. Willingness to undertake appropriate training required for the role Willingness to undertake further education/training
Desirable
6. It is essential applicants are IT literate with working knowledge of Microsoft Office applications, and able to work on own initiative and under pressure, prioritise workload and function as a team member. The ability to work flexibly is essential. Possessing NVQ 2/3 Customer service, NVQ Level 2/3 or QCF Diploma Level 2 or 3 Health Care or equivalent would be an advantage but not essential.
