Practice Based Advanced Nurse Practitioner
Through North Liverpool Primary Care Network, Aintree Park Group Practice are looking for a passionate and dedicated practice based Advanced Nurse Practitioner to join our popular and high performing practice.
You will be part of a passionate team to provide high quality services to our population of 16,200 across two sites in North Liverpool and South Sefton.
We are a dynamic and forward thinking organisation and are committed to supporting and developing our team. We have a diverse skilled workforce including a Practice Pharmacist, Pharmacy Technician, First Contact Physiotherapist and a Vulnerable Patient Care Co-ordinator.
Acute illness team comprising of 2 Acute Care Nurse Practitioners.
Dedicated Practice Nursing team and 2 Health Care Assistants supporting chronic disease management, childhood immunisations, cytology, seasonal immunisations and phlebotomy.
Main duties of the job
Job Content:
* To assess, diagnose and treat specific diseases in accordance with agreed medical and nursing protocols.
* Provide direct access clinics and telephone triage for undifferentiated patients with minor illness and ailments.
* To undertake Chronic Disease Management clinics for patients with a diagnosis of Asthma, COPD, Diabetes, Ischaemic Heart Disease, Hypertension and Rheumatological conditions under the supervision of General Practitioners and according to agreed medical and nursing protocols.
* To record contemporaneous and accurate records of delivered care on the Health Information System including appropriate SNOWMED codes.
* Participate in the share of the home visits amongst the wider team of clinicians.
* To prescribe safely, effectively and cost efficiently within boundaries of agreed competencies.
* To attend and participate in practice meetings as a nurse representative.
* Initiate and lead practice developments in first contact nursing in order to enhance the nursing contribution and quality of care.
* Provide support and training for nursing and medical students attached to the practice.
* Delegation of tasks to appropriately trained clinical and administration staff.
* Review of own requests for blood results/investigations.
