Job summary
We are looking for an enthusiastic experienced GP to join our friendly clinical team supported by a well organisedadministrative staff core. The post is flexible in sessions up to full time.The clinic is well organised with nurse prescriber and minor illness clinics,nurse led chronic disease management, onsite clinical pharmacist and socialprescriber. We have consistently achieved high QOF marks and rated good onall CQC assessments to date. Extended clinical team discussion supports staffto use the years of experience in house to inform day to day work and personaldevelopment.

Main duties of the job

Your working day will consist of face to face, telephone, video consultations, home visits and administration work.

JOB SUMMARY
The post holder will be an experienced general practitioner,who acting within their professional boundaries; will provide care for the presentingpatient from initial history taking, clinical assessment, diagnosis, treatmentand evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice.
The post holder will work collaboratively with themulti-disciplinary general practice team to meet the needs of patients,supporting the delivery of policy and procedures.

MAINDUTIES AND RESPONSIBILITIES

To be personally accountable for professionaland ethical actions and practices within a legal and ethical framework thatadheres to the GMCs requirements and local policies and procedures
To maintain confidentiality, while communicatingpatient information, in such a way that preserves the dignity and privacy ofthe patient and the patients family and carers

To promote and maintain optimum health of theregistered patient population of the Company by identifying, planning andundertaking specific health promotional activities with identified individualsand target groups in order to improve their health and facilitate furtherservice development
To establish, develop and maintain appropriatesystems for the management of patients with long-term conditions
To support the Performance Manager in ensuringdelivery on specified performance elements relating to the Company contracts, Quality Outcomes Framework, Primary Care Quality Contract, as part ofroutine administration tasks
To ensure that record-keeping is managed throughthe creation and maintenance of concise, contemporaneous computerised records,and that all care is recorded accurately and in a timely fashion
To interpret and explain complex facts inrelation to a patients condition, taking appropriate action and giving advicein a professional manner to team members, and when appropriate to family andother carers, in such a way that protects and preserves the patients dignity
To have continuing responsibility for ChildProtection and Children in Need in all cases of suspected or confirmed abuse,and to ensure compliance with local and national guidelines, policies andprocedures
To ensure that concerns and identified potentialrisks are referred to the appropriate agency immediately
To use evidence-based practice to develop andmaintain high quality clinically effective and cost-effective primary care
To assess and diagnose across a wide range ofprimary health care conditions, order investigations, plan and providetreatment as needed and discharge patients
To assess, tolerate and manage risk and dealwith uncertainty
To prescribe in accordance with local formularyand Company guidance.
To provide telephone consultations as required
