Long Stratton Medical Partnership has an excellent opportunity for a lead nurse to join our team on a full or part-time basis, for 25 to 37.5 hours per week, depending on the successful applicant.
Main duties of the job
* Providing leadership to and management of the practice nurse and healthcare assistant team, in collaboration with the GPs and practice manager in order to develop an effective, quality driven practice nursing service.
* Providing a high standard of clinical care for practice patients, delivering evidence based preventative care to the practice population.
* Implementing new clinical services and work innovatively in consultation with the GPs.
* Acting as practice lead in conjunction with other practices in collaborative working and with other secondary care services.
* Overseeing the administrative functions within the practice nurse and health care assistant & phlebotomist team. Responsibility for the nursing team's clinical functions within the GPA team.
* Providing skilled nursing services, in accordance with the NMC code of conduct, to the patients of the practice including screening, treatment & advice, within their professional competence.

Long Stratton is a small town in South Norfolk, 10 miles from the historic city of Norwich. We have 11,500 patients on our list, a CQC rating of Good and operate from both our main site and a branch site in Newton Flotman. We are also a dispensing practice with a dispensing list of around 5,500 patients.
We have completed our surgery expansion work with a new wing being added to include 7 clinical rooms. The reason for the expansion is that Long Stratton is being bypassed, 2,000 new homes are planned and therefore potentially 5,000-6,000 new patients will be joining us.

Qualifications
* A Registered General Nurse.
* Current NMC registration with an active PIN.
Experience
* Experience of working within the multi-disciplinary team.
* Possess a wide range of clinical skills.
* Experience in chronic disease management.
* Demonstrable leadership skills.
* Well organised.
* Able to work under pressure.
* Ability to use network, email and type simple documents.
* Sufficient IT skills to be able to use an electronic patient record system.
* Able to demonstrate excellent communication skills, both verbal and written.
