An exciting opportunity has arisen for an enthusiastic and experienced ANP Mental Health Practitioner to join our PCN. This role offers the chance to work autonomously while being part of a supportive, collaborative environment focused on delivering exceptional mental health care in the community.
As an ANP Mental Health Practitioner, you will:
* Independently assess, diagnose, and manage patients with a wide range of mental health conditions.
* Provide person-centred care through consultations, telephone triage, and home or care home visits as needed.
* Develop and implement evidence-based treatment plans, using assessment tools, therapeutic interventions, and CBT-based approaches.
* Prescribe medications under the Independent Nurse Prescribing Scheme (NMP) and conduct medication reviews.
* Participate in MDT meetings to coordinate care with the PCN and general practice teams.
* Engage in service development to meet increasing demands in primary care.

Main Duties of the Job
To perform as an autonomous practitioner working independently and with other health care and non-health care professionals to assess, diagnose, and treat patients with a range of mental health conditions, initiating direct referrals as appropriate.
Job Responsibilities
* Manage daily surgeries comprising face-to-face consultations and telephone triage work. There may also be a requirement for home or care home visits.
* Collaborate with GPs, nursing staff, and non-clinical support staff to make a positive contribution to the organisation and foster a One Team approach.
* Be actively involved in implementing new working methods designed to manage the increasing demand in primary care.
* Assess and triage patients, including same-day triage, and, as appropriate, provide definitive treatment or make necessary referrals to other members of the primary care team.
* Use complex decision-making to inform diagnosis, investigations, and the complete management of episodes of care within a broad scope of practice.
* Complete relevant training to provide multi-professional clinical practice and CPD supervision to other roles within primary care.
Person Specification
Qualifications
* An MSc in advanced clinical practice
* Current NMC registration as a Mental Health Nurse or other relevant professional qualification.
* Postgraduate qualification in mental health or equivalent clinical experience.

* Evidence of continuing professional development (CPD) in mental health care.
Experience
* Experience in a primary care or community mental health setting, managing a varied caseload.
* Demonstrable experience in mental health assessments, interventions, and care planning.
* Experience in contributing to service improvements or evolving care delivery in response to demand.
* Experience of working in Primary Care
* Experience working with frail or vulnerable individuals, including those in home or care home settings, is desirable.
Skills and Competencies
* Excellent communication skills: Ability to effectively engage and build relationships with patients, families, carers, and multidisciplinary teams.
* Strong clinical assessment and diagnostic skills, including mental health and physical health assessments.
* Proficient in developing evidence-based treatment plans and implementing therapeutic interventions.
* Skilled in prescribing medications under the Independent Prescribing Scheme and conducting medication reviews.
* Ability to work autonomously and make sound clinical decisions while managing risk.
* Strong problem-solving and critical-thinking skills to manage complex cases.
* Ability to prioritise workload effectively and manage time.
* Excellent teamworking skills to collaborate with multidisciplinary teams and contribute to coordinated care.
