Introduction An exciting opportunity has arisen for a Clinical Fellow in Elderly Medicine at Mid Yorkshire Teaching Trust (MYTT) on a fixed term basis for 12 months. This post will participate in the cross site senior trainee General Medicine Registrar On-Call rota. Applicants will have the same educational opportunities as other trainees and have protected SDT timetabled. These posts are not recognised UK training posts. However, the post-holders will have a named supervisor, and be supported in undertaking scheduled educational opportunities within the Trust and specialties. An Introduction to the Division / Specialty from Dr Nigel Artis - Divisional Clinical Director The Medical Division at Mid Yorkshire Teaching Trust provides Acute care across the range of medical specialities in a large modern purpose-built hospital in Wakefield, which includes a large Acute Assessment Unit, Acute Elderly Assessment Unit, Coronary Care and Acute (Respiratory) Care Unit. We also provide care for Acute Elderly Admissions and step-down care for patients in a less acute phase of their admission at Dewsbury & District Hospital. Successful applicants will support the Elderly specialty team in the management of their in-patient & outpatient workload, and will participate in the Trust-wide out-of-hours cover across both hospital sites. Our Elderly Medicine team provides a crucial role for the Trust and our local community, by striving to achieve excellent patient care and experience every time for all our patients. The team provides care for approximately 250 inpatient beds spread across 2 sites and cares for the frailest and most vulnerable in the society, as well as supporting other medical colleagues in caring for older patients. We have grown our team to be able to run dedicated Elderly Care out of hours roster separate to general medicine. Providing such varied roles also allows us the flexibility to offer bespoke careers; if you have a specialist interest that you wish to develop we can work with you to make that happe We run a broad number of specialist areas across the trust as follows: On our Wakefield (Pinderfields) site: A 40 bedded acute frailty unit (ACE) supported by a Rapid Elderly Assessment Care Team (REACT) focused on comprehensive geriatric assessment (CGA), with a view to getting patients home in a very timely fashion. A 40 bedded Orthogeriatric unit caring for frail patients following fracture neck of femur and aiming for rehabilitation transfer or leaving hospital within 5 days - the vast majority of patients now meeting best practice standards and operated upon within 36 hours of admission to our Emergency Department. A 40 bedded base ward with consultants having specialist interest in nutrition, Parkinsons disease and falls. On our Dewsbury site A 20 bedded acute frailty unit (ACE) supported by the acute frailty team, providing CGA assessment and early discharge. A 26 bedded unit which we are in the process of converting to a short stay unit Three 28 bedded elderly care wards, one taking post-operative fracture neck of femur patients for rehabilitation. We also run a large Movement Disorder Service seeing all patients over 75 with movement disorder problems with 5 clinics per week across the Trust supported by Parkinson disease nurse specialists. We also run a wide range of clinics and provide home visits to the local community. The Service Role From a clinical perspective these posts will participate in the cross site senior trainee rota. The senior trainee rota is compliant with the 2016 junior doctor contract averaging 48 hours per week. The illustrative rota pattern is attached to this job description. Duties will be based primarily on our Pinderfields Hospital site although all our medical staff are involved in delivering care to patients on all our sites. The main duties of the post will include: Provision of care with responsibility for the management of patients with acute and chronic conditions including complex cases. Referral of patients to other specialities as appropriate Out-patient activities/clinics Involvement in specialty and cross-specialty multi-disciplinary teams and meetings Involvement in relevant administration duties Teaching and supervision of junior medical staff and other clinicians as appropriate Active involvement in audit and clinical governance including service improvement activities The above list is not intended to be exhaustive and it is likely duties may be altered from time to time in the light of changing circumstances and after discussion with the post holder. The MYDoc Training Programme Educational Input & Recognition These posts are designed to help you get the experience needed for the national training program application. There are opportunities to develop your specialist knowledge and skills, lead a quality improvement project and partake in departmental teaching. Many of our previous trainees have successfully got onto a national training program with the help we have provided. Doctors appointed to a MYDoc post will receive relevant educational input. Each doctor will be allocated an Educational Supervisor and will be required to develop a Personalised Work Schedule. The weekly timetable should include a balance of these activities appropriate to your grade, role and training requirements. You are required to maintain a dialogue with your Clinical and Educational Supervisor to ensure the balance of these activities meets your developmental needs. Whilst not a recognised training post Doctors successfully completing their training programme will be awarded a Certificate of Achievement from the Trust. Doctors will use established work-place based assessment (WPBA) tools to chart progress and keep relevant evidence in an electronic portfolio system. There will be an annual review of progress based on evidence within the portfolios including the WPBA, 360 assessment, personal reflections and CPD, covering both clinical, personal and professional development, and a Certificate of Achievement (appropriate to that year/level of training) will be issued. Terms & Conditions and Tenure Employment will be as a MYDoc Clinical Fellow on Trust Terms and Conditions of Service. The initial tenure of post will be 12 months with the option to extend for a further 2 years by mutual agreement and on satisfactory performance. Appointment as a MYDoc Clinical Fellow is intended to be for a maximum three-year period as part of an individual development programme for the Doctor. Pay Scale These roles are equivalent to a doctor on specialty training programmes and will be paid on the equivalent pay scale. The salary scale commences at £61,825 per annum. Appraisal and Revalidation This post is not recognised for training and brings with it an obligation to engage with the process of revalidation on an annual basis. All our Trust appointed doctors are required to engage positively with appraisal and revalidation. The Trust has procedures that comply with national recommendations. As a Trust employed doctor on a fixed term contract successfully appointed applicants will need to have a prescribed connection with Mid Yorkshire Hospitals NHS Trust via GMC Connect and will therefore have Dr Karen Stone, the Medical Director at the Trust as their GMC Responsible Officer. Continuing Professional Development The Trust requires all its doctors to engage with continuing professional development, guided by the relevant professional associations or Royal Colleges. Time and funding is provided according to departmental and Trust policies, subject to approval through the clinical management structures. There is a programme of mandatory training which all relevant staff must complete as required. Research Medical staff are encouraged to develop areas of personal and/or research interest. This can be developed in liaison with the Trusts R&D department and in line with prevailing policies and research frameworks. The Trust is developing an expanding and outward-looking research programme. Educational & Clinical Supervision The post holder will have a named educational supervisor and be expected to hold regular meetings with them during their tenure to ensure appropriate development during their appointment. Teaching The Trust is keen to maintain its reputation for high quality training and to attract the highest calibre of trainee and career-grade doctors. All senior doctors are involved in training our doctors, students and other professions. There are many opportunities to develop training and education roles and many have taken on educational leadership roles within and beyond the Trust. Audit and Clinical Governance There is a rolling half-day clinical governance programme and audit meetings are well attended and lively. The Clinical Audit Department assists with setting and supporting an audit programme for the Service with which all colleagues are encouraged to engage. Clinical Audit is considered within objectives as part of job planning.
