The successful candidate will be anenthusiastic and committed Advanced Nurse Practitioner or Emergency CarePractitioner, with experience of working in General Practice who is able toprescribe or willing to work towards becoming a prescriber.

We are part of Farnham Integrated CareServices working in collaboration with local practices offering same day accessfor urgent care along with MSK Practitioners and Home Visiting Services.X-ray, ultrasound and phlebotomy services available within Farnham Centre forHealth. Job responsibilities
Supporting GPs by delivering a high standard of patient care usingadvanced autonomous/independent clinical skills.
To assist with same day triage, assessing, diagnosing and treatingpatients presenting with a wide range of acute illnesses.
To take part in the management and care of patients with long termconditions.
Prescribe and review medication for therapeutic effectiveness,appropriate to patients needs and in accordance to evidence based practice andnational and practice-based protocols and guidelines
Provide information and advice on prescribed over the counter medicationand mediation regimes, side effects and interactions.
Maintain a flexible approach and work alongside and cohesively withother members of the clinical team.
Duties and responsibilities:
Provide a first clinical point of contact for patients presenting withundifferentiated, undiagnosed problems. Making use of skills in history taking,physical examination, problem solving and clinical decision making to establisha diagnosis and management plan.
Patient contact may be via telephone, video, or face to face.
Instigate necessary invasive and non-invasive tests or investigationsand interpret results
Makes referrals as appropriate
Have an understanding and participate in the management of infectioncontrol within the practice
As an independent prescriber, prescribes safe, effective, andappropriate medication ad defined in current legislative frameworks
Ensure all clinical practice is safe and effective and remains withinthe boundaries of competency.
Prioritise health problems and intervene appropriately to assistpatients in complex, urgent or emergency situations including the initiation ofemergency care.
Ensures that accurate legible and timely notes of all consultations andtreatments are recorded in the patients notes.
Regularly attends clinical meetings and attend other meetings asappropriate.
Contributes to Farnham Park Health Group achieving its quality targetsand to sustain high standards of patient care and service delivery (QOF)
To make good use of IT and all software and to embrace the use oftemplates.
Professional role:
Promote evidence-based practice through use of the latest research-basedguidelines
Monitor the effectiveness of their clinical practice through qualityassurance strategies such as the use of audit and peer reviews.
Maintain professional registration
Participate in continuing professional development to ensure that up todate evidence-based knowledge and competence in all aspects of the job role ismaintained.
Actively take part in mandatory and further training as required
Work within the latest NMC Code of Professional Conduct.

Knowledge and Skills
Essential
2. Understanding of accountability to the NMC Code of Professional Conduct; and competencies of RCN and HEE Advanced Clinical Practice.
3. An understanding of safeguarding to Level 3
4. Knowledge of national standards and NICE guidelines
5. Ability to assess and manage patient risk effectively and safely
6. Excellent interpersonal, verbal and written communication skills
7. Ability to manage consultation time and prioritise workload
8. Ability to analyse data and information
9. IT skills
Desirable
10. Understanding and knowledge of primary care services including GMS/PMS contracts, QOF and clinical governance
Qualifications
Essential
11. Registered General Nurse registered with the NMC
12. Minimum of 5 years post registration experience
13. Recent experience in Primary Care, community or emergency setting
14. Evidence as working as part of a team
Desirable
15. Recognised ANP/MSC qualification at a minimum of degree level or equivalent.
16. Evidence of ability to work at an advanced level
17. Independent prescriber
18. Development of areas of clinical expertise
