Home Support Matters are looking for compassionate and motivated Reablement Support Workers to join our team in Norfolk, servicing customers in Great Yarmouth, Northern Villages, and Central Norwich. Applicants must have access to their own transport and have valid business insurance for their vehicle. This role is crucial in supporting individuals to regain independence following a period of illness, injury, or hospital admission. You will help individuals to develop the confidence and skills to manage daily activities and remain in their own homes. Block Shifts: Choose from shifts running from 07:00-14:30 and 16:00-21:00. You can sign up for as many shifts as you like, with the service operating 7 days a week. Key Responsibilities : Provide a responsive and flexible service tailored to each customer’s needs. Focus on the customer's strengths to promote independence and well-being. Support customers in rebuilding confidence after illness, health deterioration, or injury. Encourage the development of self-care skills, such as washing, dressing, cooking, and mobility. Help customers regain or retain skills, aiming for minimal or no support in the future. Prevent hospital readmission or early transition to a care home. Encourage social reconnection to reduce isolation. Work closely with customers to set goals, monitor progress, and report achievements to the Field Care Supervisor. Customer Relationships : Stand back, observe, and encourage customers as they learn or re-learn daily living skills. Collaboration : Work collaboratively with health and social care professionals to support customers in achieving their goals. Results Driven : Plan and organise tasks efficiently, aligning with the objectives of the reablement service. Take a structured approach to manage multiple tasks effectively and prioritise as needed. Requirements : Excellent communication and observational skills. Ability to encourage and motivate individuals in a positive manner. Strong organisational skills and ability to manage multiple tasks. A collaborative team player with the ability to work independently. Experience in health, social care, or a similar role is desirable but not essential, as full training will be provided.
