Patient Navigator/Administrator - Specialist palliative care team

This is an exciting post, working within the specialist palliative care team across MBHT and based in Westmorland General Hospital.
A Barrow Base will be considered if preferred. Please note this on your application.
The role will consider all aspects of the patient pathway taking into consideration all aspects of that pathway to include:
1. Ensure all patients receive appointments in a timely fashion and any other aspect of the patient pathway.
2. Support outpatient clinics, including transcribing of letters following consultation.
3. Support patient referrals to other agencies, e.g. local hospices, Cancer care, Citizen's Advice.
4. Liaise with and support patients and relatives as required on behalf of the service under the supervision of the specialist palliative care nurses or consultants.
5. Responsible for supporting the day-to-day co-ordination of the service and data management of the Specialist Palliative Care Team, including collection of and interrogation of data.
6. The post holder will work with other team members to ensure timely care and optimal service delivery.
7. The post holder will have a combined role of administrative tasks relating to the patient pathway along with some patient support and will work closely with the Team Lead for the service.
8. It is essential that the post holder exercises initiative in keeping with the needs of the role and that confidentiality is maintained at all times.
Main duties of the job
This is a blended role of administration and patient navigation, which includes supporting the Medical and Nursing staff within the service to ensure the highest level of patient experience.
The role includes the support of patient outpatient clinics, booking appointments for patients, transcribing clinical outpatient letters, supporting with data collection and patient referrals.
There is a mix of administrative tasks including the use of Excel, Word and Office 365 to support the service delivery and improvement.
The role involves working closely with the Medical and nursing staff of the small specialist palliative care team under the direction of the Team Lead.
About us
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, daycase and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Person Specification
EDUCATION
* NVQ3 in Business Administration or equivalent.
* EDCL or recognised experience.
* GCSE Maths and English or equivalent.
* Shorthand qualification.
Experience
* Experience of undertaking a range of secretarial duties, including diary management, answering telephone queries, audio typing, data collection, and word processing skills.
* Experience of using MS Office systems.
* Experience of supporting a small team of staff.
* Experience of dealing with patients/carers/families.
* Previous experience as a medical secretary in a medical setting.
* Experience of using digital dictation machines and software.
* Experience of creating spreadsheets and documents.
Skills, Ability and Knowledge
* Knowledge of data protection and confidentiality issues.
* Evidence of good, clear communication skills with good language skills.
* Demonstrates a high level of motivation.
