The West Devon Core Team are looking for an enthusiastic community support worker who is willing to learn new skills and support the rollout of our physical health clinics. The role will primarily be supporting the team to undertake physical health checks and monitoring for patients who have a severe and enduring mental health illness. While some skills in this area are desirable, full training and support will be provided for the right candidate.
The team is well established with strong managerial and clinical leadership. The right candidate will have the opportunities to undertake further training and develop a career in this field should they wish to.
Main duties of the job
* Undertake physical health checks and monitoring for patients with severe mental illness. This will be in clinics and within the community setting.
* Support qualified staff with visits and monitoring as required.
* Support patients to appointments/activities as required.
* Undertake mandatory training as required.
* Attend multi-disciplinary meetings as required.
* Liaise with GPs/primary care to ensure patients' physical health needs are adequately monitored and treated.

Person Specification
Experience
* Previous employment/experience of working with people in a caring situation.
* Previous experience working within mental health or physical health services.
* Previous care experience with people who have mental health or physical health difficulties.
* Awareness of safeguarding principles and IT skills.
* Experience working in a physical health primary care setting.
Planning & Organising Skills
* Able to plan work, manage time, complete records, and record observations.
* To take leadership and direction from qualified staff.
* To participate in unsupervised tasks and provide accurate feedback to relevant parties.
* Good IT skills.
* Ability to travel to work across a geographical area.
Qualifications
* NVQ level 3, QCF Level 3 in Health and Social Care or equivalent qualification. If equivalent qualification is not already obtained, to complete QCF Level 3 in Health and Social Care within 12 months of commencing employment.
* Other evidence of training or development relevant to working within mental health or physical health settings.

£24,071 to £25,674 a year pro rata (15 hours per week).
