Salary: Agenda for Change Band 7 £54,320 - £60,981 pa pro rata, dependent on experience (includes HCA Supplement)

It is an exciting time at Trinity as we launch our new community services, forming our clinical hub. These services aim to streamline patient care, address evolving palliative and end-of-life needs, and enhance flexibility and responsiveness.
We seek an experienced, enthusiastic, and motivated individual for the position of Triage Clinical Nurse Specialist within our First Contact team.
This role involves acting as the first point of clinical contact following patient referral. The nurse will identify and respond to immediate specialist palliative care needs through direct liaison with patients, carers, and other health and social care professionals.
Key responsibilities
1. Lead in receiving and managing hospice referrals.
2. Triage incoming referrals based on patient symptoms to determine appropriate care levels.
3. Utilize resources and guidelines to assess, prioritize, advise, schedule care, or refer patients to other services.
4. Ensure a timely, responsive, and supportive service for all stakeholders involved in referrals.
5. Manage the referral system, assessing appropriateness, urgency, and priority.
6. Collaborate with internal and external partners to coordinate patient care, ensuring it meets individual needs.
7. Use assessments and interventions as opportunities to train, coach, and mentor staff and visitors about palliative care practice.
Skills and experience required
1. Registered Nurse with bachelor’s degree in palliative/end-of-life care or related field, with extensive specialist palliative care experience.
2. Advanced assessment skills.
3. Critical thinking abilities.
4. Knowledge of disease trajectories and treatment effects.
5. Advanced communication skills.
6. Enthusiasm and flexibility.
7. Ability to work autonomously and as part of a team.
8. Ability to travel across the catchment area (a UK driving license is desirable).
