Alyn Family Doctors is a long-established group of family doctors working in partnership in the villages of Llay, Gresford and Rossett and surrounding areas near Wrexham in North Wales.
We have approximately 12,700 patients cared for by seven GP partners, two ANPs and a team of healthcare professionals. The practice operates a total triage system overseen by a GP partner each day. The administration team includes a nursing administrator and patient engagement coordinator.
We also operate a Home Visiting Service on behalf of the North & West Wrexham GP Collaborative, staffed by an Urgent Care Practitioner.
The practice is highly regarded and received excellent scores in the most recent NHS Wales Patient Experience survey.
We pride ourselves on being a friendly practice, with regular events for Hallowe'en, Christmas, and other special events! We also have book swaps and this year are business members of Theatr Clwyd, offering theatre and comedy tickets to staff.
Main duties of the job
The main duties are:
* Autonomous consulter: diagnosing, managing, prescribing and discharging patients with acute presentations and chronic conditions, both in surgery, remotely and in their own homes.
* Assessing, investigating and treating the healthcare needs of patients with undifferentiated and undiagnosed problems.
* Ordering, interpreting and acting upon appropriate investigations.
* Referring to other disciplines, to secondary care, or organising emergency admission.
* Sharing in the clinical administration duties in the practice.
* Providing clinical advice and mentorship to members of the nursing team.

Job responsibilities
JOB SUMMARY / PURPOSE
The ANP will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice and treatment to patients, carers and colleagues, ensuring clinical safety and excellence.
The ANP will work as an autonomous practitioner: diagnosing, managing, prescribing and discharging patients with illness or injury within their professional scope of practice. This may be by face-to-face consultation, telephone or in their place of residence.
The Advanced Nurse Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
The postholder will use skills, knowledge and competencies as a qualified Advanced Nurse Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admitting and discharging patients as appropriate.
This role will involve working within a multidisciplinary team to provide care and treatment to patients with either long term conditions or undiagnosed undifferentiated diagnoses.

* Share of clinical administration duties in the practice.
* Clinical advice and mentorship to members of the nursing team.
Person Specification
Experience
* Working autonomously in an independent nursing role.
* Demonstrable experience of complex decision making.
* Experience of providing care for patients with complex needs.
* Experience of working in primary care with undifferentiated presentations.
Qualifications
* Registered Nurse with Nursing and Midwifery Council.
* Masters degree OR Post graduate diploma or degree for Advanced Practice Qualification OR RCN accreditation.
* Evidence of continued professional development.
* Accredited training in chronic disease management.
* Clinical supervision training.
* Paediatric nursing qualifications are a bonus.
Abilities and aptitude
* Be able to demonstrate tact and diplomacy when working with others.
* Ability to travel between sites in a timely manner.
* Flexible approach to needs of the service.
* Experience of EMIS clinical system.
