An exciting opportunity has arisen within Hampshire and Isle of Wight Healthcare Trust to appoint an inspirational and enthusiastic HCSW to join the out of hour's service. A full supportive induction program will be provided, and you will be joining a well-established and supportive team. Working in the community you will be providing care to patients with complex care needs, enabling patients to receive the care they need in their own home. This role is very rewarding, and each shift is different with new challenges. You will be working with an experienced Trained nurse covering out of hours request for clinical review in the East Hants and Gosport area.
The work is varied and interesting including, end of life care, catheter management, falls support, Diabetes management and wound care. You will be responding directly to client needs and liaising with the Community care teams in the area. You will also be liaising with the out of hours GP services and SCAS providing care within patient's homes and care homes and responding to visits to support hospital discharges and to prevent hospital admissions as part of our urgent care response service. Please note the base for work can be in Rowans Hospice or GWMH
This position provides excellent work life balance, and we have a part time contract of twelve hours available which attracts unsocial hours enhancements. Working Twilight 17:00 until 23:30 and Night response 22:00 until 04:30 am.
Main duties of the job
Working under indirect supervision, as part of the Community Care Team, the post holder will have responsibility for delivering nursing care to patients following prescribed care plans and protocols.
Deal empathetically with sensitive information.
Working as part of the Community Care Team and with other agencies you will provide care,
following a person-centred assessment for the health and well-being of patients.
To deliver nursing care by following a care plan written as part of a holistic assessment by the registered nurse for patients and their family/carers in their own home and other community settings.
Perform basic clinical activities which will include, wound care, venepuncture, blood glucose monitoring, physical observations.
To report any changes/concerns to the registered nurse on duty in a timely fashion and record in the patient's records.
Working within the Clinical Governance framework, maintain set standards and participate Standards for Better Health monitoring.
Participation in the Community Care Team's collaboration with other agencies, statutory and voluntary to ensure continuity of care.
Actively respect patient's privacy and dignity and protect their property and possessions.
Be responsible for the ordering of stock when required as directed by the Registered Nurse.
Keep an accurate diary that reflects daily work and mileage.
Maintain legible, accurate, contemporaneous records that are dated and signed.

£24,071 to £25,674 a year Please note for part time hours the salary will be pro rata.
