NN4 PCN Manager & Digital Transformation Lead

Practices in NN4, a PCN in North Norfolk Acle Medical Partnership, Brundall Medical Partnership, Blofield Surgery, Hoveton & Wroxham Medical Centre, Staithe Surgery, Ludham & Stalham Green Surgery.
Travel between Network Practices and to other venues for meetings etc., will be required.
Primary Care Network Clinical Directors, Norfolk Primary Care & NN4 GP Practices. Local NHS organisations including ICS/ICB, local authority and local trusts, local community and voluntary organisations, and other PCN network managers, Digital Leads and locality managers.
The PCN Manager will support the PCN to develop and implement service improvement and transformation work, aligned to local and national priorities. This includes building on current primary care provision, and working proactively with the wider network to ensure we can offer our local population coordinated and more integrated healthcare. The role entails building effective working relationships, working closely with the Clinical Director, engaging stakeholders across different organisations and disciplines, coordinating work-streams, collating and monitoring risks and issues, building consensus across the six practices of NN4.
The PCN Manager will coordinate the successful delivery of all contractual requirements.
Main Duties of the Job
The PCN Manager & Digital and Transformation Lead responsibilities include but are not limited to the following:
1. Manage compliance with the Network DES specification on behalf of the NN4 PCN member practices, including:
1. Achieving maximum achievement to meet Investment and Impact Fund (IFF) targets and other network-related achievement schemes.
2. Producing performance and quality reports within the Network.
2. Coordinate and report on Quality and Outcomes Framework (QOF) Quality Improvement (QI) projects across the PCN.
3. Work with the Clinical Director and Board to develop and deliver detailed workforce plans, aligned to PCN priorities, and reflecting opportunities within the Additional Roles Reimbursement Scheme (ARRS).
4. Work with the Clinical Director and Board to develop and deliver a continuous improvement approach to digital accessibility for our patients and workforce, including supporting national and local initiatives.
5. Manage the staff performing ARRS-funded roles deployed across the PCN to ensure satisfactory leadership and clinical supervision is in place.
6. Support recruitment, induction and training of new PCN staff where required.
7. Work closely with other service providers engaged to deliver services under the ARRS scheme.
8. Support the Clinical Director and associated decision-making at formal PCN and informal meetings.
9. Manage PCN-related projects.

Previously North Norfolk Primary Care, the company was incorporated in April 2017, with the vision to support local primary care. NPC now employs more than 170 staff. NPC supports seven Primary Care Networks (PCNs) across Norfolk and Waveney.
PCNs form a key building block to the NHS Long Term Plan, enabling GP practices to work together at scale and provide greater proactive, personalised, coordinated, and more integrated health and social care for people close to home.

Skills and Abilities
* Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face.

* Excellent interpersonal, influencing and negotiation skills organisation skills with the ability to constructively challenge the view and practices of managers and clinicians.

* Ability to use own initiative, discretion and sensitivity.
* Full driving license.
Experience
* Minimum of 2 years experience of working with healthcare professionals and/or previous experience in the NHS or social care or relevant field.
* An appreciation of the nature of GPs and general practice. An appreciation of the nature of primary care prescribing.
* Experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.
* An appreciation of the new NHS landscape including the relationship between individual providers, PCNs and commissioners.
* Experience of Mental Health related care of patients.
* In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
* Understanding of the mentorship process.
* Understanding of the implications of national priorities for the team and service.

* High level of verbal, written and numerical literacy.
* Self-development through continuous professional development activity.

Depending on experience 21 hours, Salary £22,742.76 / FTE £40,612.07
