Job summary We are looking for individuals who can demonstrate attention to detail to join our Referrals Management Team. You will ensure that the flow of referrals and reports flowing in and out of Nexus is maintained to a high standard. You will also support us in maintaining positive and effective information sharing across Nexus, and ensure patients are informed of outcomes as directed by our clinical team. Main duties of the job To ensure that referrals received are processed in a timely manner according to the training given and within the appropriate time scale. Provide support to patients, practice staff and hospital/community trusts regarding referrals. To gain and utilise good knowledge of referral pathways Process requests for reports and/or copies of medical records To ensure that referrals received are processed in a timely manner and in accordance with the designated pathways, practice policies and processes within the appropriate timescales. About us Nexus was formed in 2016 with the merging of a number of established local practices in north Southwark.We look after a practice population of 76,000 patients spread across our 9 sites. The overall aim of Nexus Health Group is to provide a clinically excellent service to our patients, recognising the needs of our local population and to provide a learning and developmental environment for our staff. Our 9 sites across north Southwark are geographically close and well served by local transport hubs at London Bridge, Elephant & Castle and Canada Water.We share a single patient list but our patients are attached to a specific site and are looked after by local multidisciplinary teams which include GPs, Nurses, Physicians Associates, Paramedics, ANPs and Pharmacists. These site based teams are supported by Managers and administration staff, as well as centralised HR, IT, Clinical Administration and the Senior Management Team. All our sites function as traditional practices with a Site Manager and GP partners working in the practice. All sites are involved in GP training and increasingly training the wider AHP team such as nurses and PAs. Nexus works closely with the local PCN with 2 partners as Clinical Directors.
