This is an exciting opportunity to join a vibrant, mature and constantly evolving Primary Care Network (PCN) in the Pudsey / Bramley area (multiple sites) as part of a well developed, mixed skill pharmacy team. PCNs formed part of the NHS Long Term Plan published in 2019 and are tasked with providing enhanced clinical services across multiple GP surgeries, servicing and benefitting the entire patient population on an economies of scale basis, whilst also relieving pressure on practice staff. Undertake medicines reconciliation in accordance with practice protocols & update patients medical records accordingly;respond to medication queries from patients & staff in a professional manner referring to the clinical pharmacist when appropriate; accurately & promptly action hospital discharge / treatment notes; support the PCN multi-disciplinary team to ensure efficient medicines optimisation processes are followed;promoteElectronic Repeat Dispensing (eRD) / online ordering; develop relationships with other pharmacy professionals & members of the multi-disciplinary team to support integration (primary care, community pharmacy, secondary care & mental health); support practice reception teams in stream-lining general prescription requests, so as to allow GPs & clinical pharmacists to review the more clinically complex requests

West Leeds PCN provides services for 6 practices in and around Pudsey / Bramley with a total patient population of approx. 70,000. We have a mature workforce covering many clinical disciplines (Occupational Therapists, Social Prescribers, Physiotherapists, Paramedics, Dietitian, Health Care Assistants, Health & Wellbeing Coaches, Physician Associates, Care Co-ordinators, Mental Wellbeing Practitioners) plus a pharmacy team comprising 4 Senior Pharmacists, 3 additional Pharmacists and 2 Pharmacy Technicians. We also mentor pre-registration pharmacists which means there is a wealth of clinical support available within your own team

Job responsibilities
Core Responsibilities - Clinical
o To undertake medicine reconciliations inaccordance with general practice protocols and to update patients medical records / prescribingaccordingly.
o As determined by the PCN, support medicationreviews and medicines reconciliation/synchronisation for groups of patients new care home patients
o Support initiatives for antimicrobialstewardship to reduce inappropriate antibiotic prescribing locally.
o To respond to medication queries from patientsand staff in a professional manner referring to the clinical pharmacist whenappropriate.
o Process hospital correspondence such as eDANS / TANs
o Action MHRA / supply shortage alerts
o DOAC / statin / hypertension reviews
Core Responsibilities Technical andAdministrative
o Support the PCN multi-disciplinary team toensure efficient medicines optimisation processes are being followed.
o Implement efficient ordering and returnprocesses and reducing medication wastage.
o Promotion of Electronic Repeat Dispensing(eRD) and online ordering.
o Develop relationships with other pharmacyprofessionals and members of the multidisciplinary team to support integrationacross health and social care including primary care, community pharmacy,secondary care and mental health.
o Support practice reception teams instream-lining general prescription requests, so as to allow GPs and clinicalpharmacists to review the more clinically complex requests.
o Support the implementation of nationalprescribing policies and guidance within GP practices, care homes and otherprimary care settings.
o Support the PCN to deliver on QIPP agenda, QOFand locally commissioned enhanced services
o Support the PCN in reviewing and developingpractice policies for CQC requirements.

Person Specification
Qualifications
Essential
1. BTEC Level 3 Diploma in Pharmaceutical Science or
2. NVQ Level 3 Diploma in Pharmacy Service Skills or equivalent with underpinning knowledge.
3. Current registration as a Pharmacy Technician with the General Pharmaceutical Council.
4. Evidence of commitment to continued professional development in line with regulatory requirements
5. Must be prepared to complete the CPPE General Practice Pharmacy Technician Training Pathway
6. Must be willing to participate in any relevant training identified to develop skills needed to carry out the role
Desirable
7. BTEC Level 4 Diploma in Pharmaceutical Services
8. Post-registration training in specific areas of pharmacy care homes, or clinical areas inhaler check training
