An exciting opportunity has arisen for an Enhanced Clinical Practitioner to join the Advanced Practice team within the Purbeck Hub delivering care to the residents of Purbeck. We welcome applications from allied health professionals and nurses.
As part of the Network, you will be joining a multi-disciplinary service to provide urgent community response and admission avoidance to our patients whilst working with the team on service delivery, triaging and prioritising visits to achieve our overall objectives.
You will be based within the Purbeck Hub, Wareham along with an excellent team of Practitioners, who are supported by a GP, a hospital consultant and long-term condition HCAs.
You will provide clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and decision-making skills. Your role will be community-based supporting people in their own homes.
This is a fixed-term contract for 1 year covering maternity leave. Hours are part-time at 22.5 per week between 8.30 am and 5 pm; as the service develops, shifts may cover 8 am - 8 pm with possible weekends.
This is a fixed-term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
Main duties of the job
1. To provide high-quality person-centred care which always considers people's safety, privacy and dignity using enhanced clinical knowledge that is evidence-based.
2. To undertake independent assessment and management of service users to incorporate pharmacological considerations, where appropriate making complex clinical decisions regarding service user management and clinical outcomes.
3. To provide enhanced clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and enhanced decision-making skills.
4. To manage change, to monitor and improve standards through supervision, evidence-based practice, clinical audit, research, and education. To promote and demonstrate best practice by integrating evidence into practice.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Person Specification
Knowledge, Skills and Training
* Registered practitioner to degree/diploma level supplemented by minimum PG dip (120 credits) plus MA/MSc or working towards in a relevant subject or demonstrable extensive experience in the relevant specialty
* Learning and Assessing in Practice Qualification or equivalent practice assessors training
* Non-Medical Prescriber (if professionally appropriate)
Job Specific Experience
* Experience at Registered Practitioner Band 6 level
* Experience of developing specialist programmes of care for an individual or groups of patients/clients and of providing highly specialist advice
* Evidence of involvement in the development of programmes of care, protocols and audit.
* Trained in PMVA techniques or willing to be trained.
* Evidence of proactive involvement in the supervision and development of members of the multi-disciplinary team.
* Evidence of delivering education and training in practice.
Information technology
* Experience of using electronic patient/service user record systems
Personal Qualities/Attributes
* Able to communicate effectively at different levels of the organisation and with staff, patient/service users, visitors or external organisations both verbally and in writing in the exchange of highly complex, sensitive or contentious information which may require the use of negotiating and/or persuasive skills.
