Job Description
Job Title: Health & Well-being Worker
Department: Venture
Salary: £24,424pa (pro rata £14,654pa)
Hours of work: 22.5 hours per week (days/times to be agreed)
Contract Term: Initially fixed term for 1 year with the possibility of extension (Subject to a 6 month probationary period)
Job Purpose: To support and empower individuals and communities to make positive lifestyle changes, improve their health outcomes and enhance their overall well-being. This role involves outreach, health education, and working collaboratively with local organisations and healthcare providers to address barriers to healthcare and promote health equality.
Duties and responsibilities:
All staff employed by the Trust have an individual responsibility for promoting and safeguarding the welfare of the children, young people and vulnerable adults whom they are responsible for or come into contact with.
1. Community Engagement and Outreach:
o Build relationships with local communities, to understand their health needs and barriers.
o Develop and identify initiatives to overcome barriers, promote well-being workshops, and informational sessions.
o Work with local organisations and stakeholders to promote health-related events and services.
2. Health Education and Promotion:
o Design and deliver health education sessions focused on preventive care, nutrition, physical activity, mental health, and other relevant topics.
o Create and distribute educational materials that are culturally appropriate and accessible for different populations.
o Conduct one-on-one health coaching sessions to encourage healthy lifestyle choices.
3. Program Development and Implementation:
o Collaborate with healthcare providers, the Family Hub and local community organisations to identify and address public health issues.
o Develop programs that address health inequalities and promote well-being within specific communities (e.g., women's health, mental health, nutrition).
o Monitor and evaluate program effectiveness, adjusting strategies as needed to meet community needs.
4. Data Collection and Reporting:
* Collect and maintain data on client interactions, community needs assessments, and program outcomes.
* Prepare reports to track progress, demonstrate impact, and inform stakeholders and funders.
* Use data insights to inform program development and prioritise health initiatives.
1. To be familiar with policies and procedures of the Trust and uphold them, especially those relating to Safeguarding and Child Protection, Health and Safety, GDPR and Equalities.
2. To participate in the Trust’s performance management scheme.
3. To undertake personal professional development and training as appropriate.
4. To assist with organising and running community events in Balsall Heath as appropriate.
5. To participate in the supervision process including the Probationary Review.
6. To undertake any other duties commensurate with the role and responsibilities of the post as agreed with your Line Manager.
