Orchard Surgery has an exciting opportunity for an experienced nurse to join our friendly practice. A fantastic multidisciplinary team will support you, comprising of GPs, paramedics practitioners, clinical pharmacists, practice nurses, HCAs, care coordinators, a health coach, and we are currently recruiting a social prescriber. All staff have the opportunity to extend their roles and develop new skills. There are opportunities for leadership and specialization. Some of our care navigators are trained to do ECGs and urine dips, freeing up our nurses to undertake more specialized care. We are currently looking at the role of group consultation for diabetes and other LTCs, having successfully implemented menopause group. Nurses have regular meetings as a nursing team, fortnightly clinician meetings in practice, and collaborative nurse-led meetings in the PCN. You will also have access to regular mentoring and clinical supervision.
Orchard Surgery is part of the new Park and Orchard PCN in Horsham. We work collaboratively together and with our local community in partnership. Our current focus is on improving proactive care for our frail elderly housebound patients, and access to women's health is a strong focus on staff wellbeing in the practice. We close the practice front doors for 1 hour at lunchtime to allow staff to take a break, have lunch together, or go for a walk. We also offer training sessions at lunchtimes.

We have strong teams of ARRS roles, which contribute to the care we can offer our patients and help reduce the demand on our practice staff.
Job responsibilities
1. Identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition, as appropriate
2. Work with other health care professionals to diagnose, monitor, manage & treat long-term conditions including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
3. Infection control policies, training and implementation
4. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols with GP support
5. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
6. Support patients to adopt health promotion strategies that promote healthy living, and apply principles of self-care, utilising community assets such as the Horsham Wellbeing Hub
7. Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment
8. Participate in planning and implementing changes within the area of care and responsibility
9. Provide assessment, screening and treatment services and health education advice
10. Participate in organising and co-ordinating the provision of nursing services for the practice to include the delegation of duties to other practice team members as appropriate
11. Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols
12. Provide general and specific health screenings to the practice patients- within agreed protocols with referral to general practitioners as necessary
13. Advise patients on general health care and minor ailments with referral to GPs as necessary
14. Deliver vaccinations and immunisations as required
15. Meet the needs of patients presenting for wound care
16. Ensure the maintenance of equipment and stock levels relating to patient care
17. Undertake to support the administrative team in the collection, storage and dispatch of pathological specimens including swabs, smears etc.
Administrative and professional responsibilities
1. Participate in the administrative and professional responsibilities of the Practice team
2. Ensure accurate notes of all consultations and treatments are recorded in the patients notes and on the clinical computer system as appropriate
3. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice
4. Ensure collection and maintenance of statistical information required for regular and ad hoc reports, audits and research programmes
5. Attend and participate in practice clinical meetings on a regular basis
6. Assist in the formulation of practice philosophy, strategy and policy and develop appropriate protocols
