We are seeking a dedicated and caring Practice Nurse to join our healthcare team. As a Practice Nurse, you will play a crucial role in providing high-quality patient care. This is an excellent opportunity to join our busy practice. Essentially we are looking at between 25-30hrs per week but this can be negotiable for the successful applicant.
Main duties of the job
Applicants should be able to work independently on all practice matters and take responsibility for general practice issues on a day-to-day basis. They should be able to provide excellent care to all our patients. Good communication and organisational skills are essential. Awareness of QOF, CDM and local requirements are also needed.

Fir Park Medical Centre is a busy GP practice in Widnes with around 12,200 registered patients. We are a training practice with 7 GPs, 2 ANPs, 2 Clinical Pharmacists, 3 Practice Nurses & 2 HCAs. We moved to purpose-built new premises in November 2021, which will offer expansion opportunities.

Job responsibilities
The successful applicant will be responsible for the delivery of basic practice nursing services care to the practice population. They will deliver care within the boundaries of their role, focusing on supporting patients to be healthy, monitoring of long-term conditions, health prevention, and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, well women and well man clinics, as well as actively supporting the practice team in the reviewing and delivery of clinical policy and procedure.
