We have an exciting new opportunity for 2 Band 4 Dietetic Assistant Practitioners (fixed term for 12 months) to join our community Nutrition and Dietetics team. This post is ideal for someone who is enthusiastic and self-motivated, with an interest in developing their knowledge and skills in nutrition support.
This role is responsible for working with care homes for older adults and supporting the delivery of an innovative project involving training, education, audit, clinical reviews and report writing and presentations.
The service is a successful and proactive team of dietitians and dietetic assistant practitioners covering Westminster, Kensington & Chelsea, and Hammersmith & Fulham boroughs. We deliver nutrition support, home enteral feeding and community gastroenterology to the wider community. We are a supportive and innovative team that continuously seeks to adapt and improve the service we provide and a strong focus on the health and well-being of our staff.

The team is predominantly hybrid working, meets face to face once a month and virtually weekly. The postholder will join a wider network of Dietitians and Dietetic Assistant Practitioners across the Trust. There is a strong emphasis on learning and development.
1. You will manage a caseload of approximately 20 care homes (residential and nursing homes) to deliver population health improvement strategies.
2. You will conduct audits in care homes to identify improvement needs.
3. You will deliver training and education with a focus on the care homes improvement needs.
4. You will actively promote a food-based approach to managing malnutrition and reduce reliance on prescribed oral nutritional supplements.
5. You will utilise influencing skills to embed the British Dietetic Association’s Care Home Digest within the care homes.
6. You will support student training.
7. We will support you to actively participate in clinical supervision, continuous professional development, and the development of leadership skills.
