Job summary We looking for qualified or trainee Advanced Clinical Practitioners with non-medical prescribing and advanced adult health assessment qualifications to join our team working across GEMS (Geriatric Emergency Medicine Service), Orthogeriatrics and our Acute Frailty Unit (AFU). Our team is an enthusiastic and motivated multi-disciplinary team of Advanced Clinical Practitioners, Pharmacists, Nurses, Physiotherapists, Occupational Therapists, and Social Workers who work alongside Consultant Geriatricians and Non-medical Consultants to deliver specialist frailty services,both at the busy front door of the acute hospital and also to our inpatient areas ensuring a consistent frailty sensitive ethos is maintained.
