Key Areas of Responsibility To process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary. To document triage notes in order that the patients record is complete and for audit purposes. To use information technology to process the referrals for the 80% triaging role. To liaise and communicate with the general public, other health care professionals, and multi-agencies on a daily basis. To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible. To undertake unplanned care in conjunction with community nursing teams. To support community nursing teams with provision of care in catheter clinics To work within the integrated team to facilitate early discharge from hospital. To work within the integrated team to prevent unnecessary admission to hospital. To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
