We are looking for a Community Nurse to work in and around the St Ives and Hunts locality, you will be fully supported to gain new skills and experience needed to deliver high quality nursing care to patients in their own home or residential setting.
As a community nurse, we will develop your wound care, palliative care, and long-term conditions, knowledge, and skills; supporting you to become an expert in assessment and care-planning to deliver quality care that is caring and compassionate.

Main duties of the job
We are looking for people who:
1. Have experience of working in the Community
2. Are educated to RGN level
3. Are able to demonstrate a commitment to high quality care and service provision
4. Have the ability to work flexibly and as part of a team
5. Have the ability to assess and formulate care plans
6. Are able to teach and support student Nurses and Health Care Assistants
7. Have enhanced communication and inter personal skills
You will be responsible for:
8. Keeping the patient at the centre of care, ensuring patient and carer participation in decision making
9. Providing a high standard of care
10. Providing a high standard of nursing intervention within a patient's own home, with access to Specialist and Advanced Practitioners when required
11. Working as part of a / service and multidisciplinary team
