Develop or already be experienced in specialist nursing skills in providing additional care for long-term conditions and diseases such as asthma/COPD care, diabetes, cancer care reviews etc Deliver care according to current guidelines including the NMC, National Institute for Clinical Excellence (NICE) guidelines, Chief Medical Officer guidance and evidence-based care. Ensure that the clinical working environment is organised, safe and efficient - as a minimum, to meet CQC requirements. Support the Practice to achieve its requirements within the QOF framework, enhanced services, targets and other policies and protocols. Continue to develop other clinical competencies as agreed with the Practice. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
