Job summary Fixed term role for 23 months 22.5 hours per week To assist qualified staff to provide assessment support to children and families in the Child Development Centre To act as key worker and support for children and families attending the Child Development Centre To link families to services identified through the CDC assessment To be part of the multi-disciplinary team and undertake specific support work as supervised by the Therapy Teams at the Child Development Centre To work in collaboration with the child and parent/carer to implement programmes of support and intervention The role is based in the Child Development Centre at Kimberworth Place but may involve community working within homes, nurseries and mainstream schools for all children aged 0-5 years. Main duties of the job To work as a member of the multi-disciplinary interagency team establishing effective communication with colleagues, child and families. Will provide assessment support to the child and family undergoing assessment Will assist the CDC staff to provide a family centred service working as key worker within the CDC to ensure families are supported. Will support the family after the initial assessment, follow up appointments and key stages of transition. Will assist therapy staff in the assessment of the children both at the CDC, home and in education settings. To be the key worker and link between the CDC service and outside agencies Will attend team around the child meetings and will facilitate the families to access Early Help Work under direct supervision of the therapy teams to assist the assessment process.
