£32 ph 3 months We are looking for an experienced Social Worker to work on site closely with Health colleagues to facilitate a safe return home for Vulnerable adults who present in the Emergency Department at the Princess Royal University Hospital in Farnborough, Bromley. You will be required to complete assessments under the Care Act 2014 and Mental Capacity Act 2005. Also arranging care support to enable adults to return home to reduce the risk of social admissions to hospital or temporary placements to manage initial risks until further assessments can take place. You will attend daily board meetings to discuss patient flow and plan the adult’s return home. Once the adult has returned home the case will be followed up by the respective Locality teams. You will have a knowledge/experience of facilitating safe Hospital discharges, strong working knowledge of the Care Act 2014 and its application including Section 42 Safeguarding threshold. Timely strength-based assessments Also, knowledge of the Mental Capacity Act and experience completing Mental Capacity assessments. You will be supporting Younger Adults with physical disabilities and sensory impairment and Older People to live as independently as they are able and work with their carers to ensure risks are appropriately managed.
