The UHS Heart Failure Team is a specialist multidisciplinary team that provides care and management of patients with heart failure within UHS as well as in the community in Southampton and South West Hampshire. The team comprises of Specialist Nurses, Cardiology Consultants, a Palliative Care Consultant and a Pharmacist. We work collaboratively with colleagues cross care groups within UHS and the Hampshire and Isle of Wight ICB to provide evidence based, patient centred care to improve prognosis, symptoms and quality of life. Ideally the candidate will have experience of working in Cardiology and have a history taking and physical examination qualification. This is not essential however and training can be undertaken. Phlebotomy skills are desirable. Demonstrable clinical competence, excellent communication skills and a team ethos are essential. The nurses are hospital or community based and have the opportunity to rotate between the two areas therefore a driving license and ability to travel are essential. The role involves assessing, treating and educating heart failure patients on the ward as well as in an outpatient setting. This may be within UHS or Community clinics, therefore having the ability to travel across multiple sites is essential. As well as face to face reviews, the service is developing the digital aspect of it's care, building on the utilisation of digital technologies, including the integration of My Medical Record to provide safe and effective care in up-titration of prognostic medications.
