We are a practice team recognised for our great team morale and high QOF achievement situated in a semi-rural location in Calverton, Nottingham. Our patient demographic is mixed allowing for the successful candidate to utilise current skills and develop their potential.
Main duties of the job
We are looking for the right candidate to fit in with our current nursing team and would be interested to hear from experienced candidates particularly with interests in asthma/COPD management.
About us
The Calverton Practice is a long-established dispensing GP Practice covering Calverton and the surrounding villages with a list size of approximately 10,000. We have a team of 6 GP partners, 3 salaried GPs and as a training practice host both GP registrars and medical students. Our nursing team currently consists of 4 nurses, a paramedic and 3 HCAs who are supported by staff from across our local PCN including pharmacists, physiotherapists, and mental health OTs. We also have a large, supportive administration/managerial team. As well as working in a great team, a recent extension and refurbishment programme provides an excellent environment in which to work.
Job responsibilities
1. Provide routine and acute nursing care to patients in accordance with clinical based evidence, NICE and the NSF.
2. Diagnose and manage chronic conditions, support the provision of chronic disease clinics, delivering patient care as necessary.
3. Provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages and prescribe medication for patients in accordance with PGDs and own scope of practice.
4. Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans etc.
5. Prioritise health issues and intervene appropriately, delivering opportunistic health promotion where appropriate.
6. Support the team in dealing with clinical emergencies.
7. Support the implementation of vaccination programmes for adults and children and support the provision of travel medicine services.
8. Contribute to practice targets (QOF etc.), complying with local and regional guidance and participate in local initiatives to enhance service delivery and patient care.
9. Support and participate in shared learning within the practice to improve patient care.
10. Continually review clinical practices, responding to national policies and initiatives where appropriate.
11. Maintain professional registration and adhere to revalidation requirements.

Qualifications
* Post graduate diploma or degree (relevant to role).
* Qualified Nurse prescriber.
* Qualified Triage Nurse.
Experience
* Experience of working in a primary care environment.
* Experience of working autonomously.
* Experience of infection prevention and control measures.
* Experience of working as a practice nurse or community nurse.
* Experience of chronic disease management.
* Experience of prescribing and undertaking medication reviews.
Clinical Knowledge and Skills
* Wound care including removal of sutures and staples.
* ECGs.
