An exciting opportunity has opened for an Advanced Nurse Practitioner to work within our Practice, Bowland Medical Practice in Wythenshawe, South Manchester.
We are looking for an experienced nurse acting within professional boundaries to provide care for patients from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
The post holder will also undertake general treatment room duties as demand dictates and participate in chronic disease management.
They will be responsible for same day and planned patient care needs, minor or complex, in primary care.
To support the practice to ensure compliance with its statutory and regulatory obligations, such as but not limited to the Care Quality Commission and Data Protection Act 2018 (GDPR).
Main duties of the job
1. Assess, diagnose and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team.
2. Ensure clinical practice is safe and effective and remains within boundaries of competence, acknowledging limitations to the team for appropriate referrals.
3. Assist in telephone triage of urgent cases.
4. Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary.
5. Undertake Learning Disabilities reviews both in the practice and when home visiting.
6. Collect pathological specimens including intravenous blood samples, swabs, etc.
7. Perform investigatory procedures requested by the GPs.
8. Visit housebound patients as agreed with their GP or the on-call team.
9. Maintain confidentiality of patients' records.
10. Make effective use of the IT system EMIS by ensuring consistent and accurate entry of data and codes, ensuring all contacts with patients are legibly recorded.
11. Contribute to practice/team audit/research as required.
12. Support the equality, diversity and rights of patients, carers and colleagues.
13. Follow practice infection control procedures, promoting and maintaining appropriate health & safety standards in line with the practice Health & Safety Policy.

Person Specification
Qualifications
* Prescribing Qualification
* Higher study in clinical practice and Advanced Level Clinical Assessment Reasoning and Decision Making
* Diabetes, asthma, CHD, COPD diploma or equivalent
Experience
* Proven employment history which supports the application of sound clinical knowledge.
* Primary Care or Urgent Care Environment experience (including Minor Illnesses)
* Basic Life Support certification from the Resuscitation Council or equivalent within the last 12 months.
