To work as a member of the Intermediate Care Team central Manchester, providing direct care for patients in the community who have been referred to the service
The team are multi-professional and multi-agency and provide assessment and rehabilitation. Their aim is to maintain people in their own homes, prevent re-admission to hospital or inappropriate admissions to residential or nursing homes, by maximising function and independence. Care is delivered in a variety of settings including units and patients own homes and their community.
The post holder will follow agreed protocols for ensuring appropriate and timely input, and ensuring pathways of care and communications with all agencies working with the service ensure continuity of care whilst respecting patient choice and autonomy.

The service provides care to people in their own homes and in dedicated Intermediate care beds, both nursing and residential within Gorton Parks nursing home. You will work in people’s homes and in the bed base so it is essential that you can travel independently within the local geographical area to meet the service needs. You will be required to work weekends and late shifts on a rota basis, with late shifts being from 12 noon until 8pm.

Experience of working in the NHS and experience of working in a rehabilitation/ community setting are desirable.
