Job responsibilities
Key responsibilities:
Establish contact with patients/carers from thetime of initial referral to provide specialist nursing advice and practical andpsychosocial support.
Holistically assess and identify health andpsychosocial needs of individual patients, and plan programmes of care to meetthese needs, in partnership with patients/carers and colleagues.
Manage a patient caseload, to agreed referral /discharge criteria, and act as key worker.
Deliver knowledgeable nursing care to patientsin agreed practice settings.
Maintain adequate patient documentation to NMCrequirements.
Contribute to the development of nurse-ledinitiatives for the benefit of the identified client group in relevant caresettings, in conjunction with medical and nursing/AHP colleagues.
Workin collaboration with health and social care colleagues within the Trust andexternal primary and acute care providers to develop patient-centred pathwaysof care for smooth and timely referral, minimising delays and supporting sharedcare.
Person Specification
Experience
* Registered nurse on relevant part of the NMC register
* Degree qualification, 120 credits at L6 or possess an equivalent knowledge to be assessed through evidence of relevant specialist training, experience or portfolio
* Significant post registration experience / qualification / training, including experience and knowledge within relevant speciality
* Completion of all Band 5 (preceptorship) competencies
