Weare looking for an enthusiastic and motivated individual to join our team and support our member practices to increase the uptakeof childhood immunisations and promote NHS immunisation campaigns.
NorthHalifax PCN is a well-established network of 4 practices, who have workedcollaboratively delivering care to our population of over 38,000 patients, evenbefore the networks were created in 2019.
Thepost holder will work on a rota basis within practices and at our PCN hub in thenew purpose build Mixenden Community Hub.The successful candidate will be required to travel independently between practice sites. Thecandidate will require a flexible approach to change and be instrumental in thedelivery of the practice/PCNs aims for patient centred care.
Thisvacancy is for 30 to hours.

Job responsibilities
15. The following are the core responsibilities. Theremay be, on occasion, a requirement to carry out other tasks; this will dependon factors such as workload and staffing levels:
16. Undertake responsibilities in line with practicedemand for childhood vaccinations
17. Provide education and advocacy about the benefitsand safety of vaccinations, addressing any concerns in misconceptions individualsand their carers
18. Work with practices to facilitate easy access toimmunisation services
19. Track immunisation rates and follow up withfamilies.
20. Build and maintain relationships with patients andfamilies with consideration to health inequalities and diversity
21. Participate in outreach activities to raise childhoodvaccination awareness and uptake
22. Work closely with other health, social and educational care professionals,such as local schools, childcare providers, to identify and support patientsmaking sure that access to appropriate vaccinations is available to them andensuring that their needs are met
23. Report on progress, challenges and outcomes to managementand stakeholders
24. Proactively identify and work with registeredpatients, their families and carers to support their personalised care needs inorder to achieve vaccination uptake
25. Ensure that people have good quality informationto help them make educated choices to vaccinate.
26. Signpost support to patients/family/carers oninformation about immunisations
27. Maintain accurate and contemporaneous patientrecords at all times
