Main duties of the job Rapid Response is a service which aims to meet patients' urgent care needs at home by providing urgent clinical, therapy or support interventions to keep a patient in their place of residence, which is key in improving patient outcomes, preventing avoidable hospital admissions and delivering NHS strategic priorities. The service is part of the new Urgent Community Response agenda and will respond within two hours of a referral to meet the UCR national guidelines. The RGN will provide and maintain a high standard of evidence based, patient centred care. To demonstrate good interpersonal skills and to develop practical skills required within the speciality, whist reflecting the philosophy of care. To deliver patient care on a daily basis, using the systems in place in the department for a multi-disciplinary approach to patient-centred care delivery, which builds on the philosophy of greater involvement of patients and their relatives/carers by using the Trusts patient and public involvement strategy.
