The Tissue Viability Nurse is an integral part of the Harm Free Care Team to reduce pressure ulcer incident rates and improve patient experience with wound care.
Under the direction of the Tissue Viability Nurse Specialist, the Tissue Viability Nurse will deliver evidenced based, high quality wound management and pressure ulcer prevention to ensure the best outcomes for patients.
The post holder will need to be able to assess the patient holistically and act as their advocate to achieve optimal wound healing goals.
Support the Tissue Viability Nurse Specialist to develop and provide specialist training across UHSussex in line with Trust and National requirements.
Works with the Tissue Viability Nurse Specialist and HFC Team to develop a culture of learning from incidents.
The post holder will have delegated responsibilities including supporting and developing high quality evidence based wound care practices within the organisation.
The post holder will assist with the development and participate in the delivery of the tissue viability educational program to health care professionals involved with wound care and skin care within the acute care setting.
Main duties of the job
Provide specialist clinical knowledge and holistic advice to the multi-professional teams.
Promote and develop the use of evidence-based practice in tissue viability to improve the outcomes of patient care.
Work both collaboratively and autonomously under the direction of the Tissue Viability Nurse Specialist to make clinical decisions using expert knowledge and advanced assessment skills to provide the highest standards of holistic care.
Job responsibilities
* Perform comprehensive advanced holistic assessments of patients' nursing needs, plan, implement and evaluate care delivery according to changing health care and tissue viability needs.
* Deliver specialist clinical care, with responsibility for assessment primary and/or secondary diagnosis within an agreed scope of practice.
* Management and evaluation of treatment and/or care for patients with complex wounds supported by the Tissue Viability Nurse Specialist.
* Make clinical judgements about patients and adjust treatment plans accordingly to achieve treatment targets.
* Deliver expertise in triage, wound assessment and planning of care to ensure patients with more complex wounds are reviewed in a timely manner.
Person Specification
* Experience in effective wound management and pressure ulcer prevention.
* Acute Nursing Experience.
* Mentorship evidence in CPD.
* Tissue Viability course.
