We have excellent opportunities for you to work in Community District Nursing Services within the central and west locality, with posts available in Greater Preston and Chorley and South Ribble. Working hours are from 07:45-22:00.
As a member of the team, you will provide holistic, high quality, clinically effective care to patients, working under the guidance and support of the Team Leader and Team Manager. If you are a Registered Nurse or are considering returning to practice after a career break or following retirement, this is the perfect opportunity to engage directly with our nursing teams across our community. Main duties of the job
Community Nurses are highly skilled practitioners who carry out interventions which include:
1. Holistic assessment of needs
2. Wound care
3. End of life and palliative care
4. Lower limb assessment and management
5. Diabetes management
6. Bladder and bowel care
7. Safeguarding/ MDT working
8. IV therapy
9. Admission avoidance
10. Discharge facilitation
11. Sepsis screening
The successful candidate must demonstrate excellent clinical and communication skills to provide a high standard of care to ensure a positive patient experience. Experience in community care is an advantage; however, the post is also suitable for newly qualified nurses. We have an excellent preceptorship programme which all our newly qualified staff will follow.

Person Specification
Qualifications
* First level registered nurse/newly qualified nurses would be considered for the post
* Diploma in health related area or equivalent experience
* Completion of Band 5 preceptorship programme (evidence may be required from RNs who have been registered for less than 2 years or with gaps)
Knowledge
* Understanding of NMC standards and implications for practice
Experience
* Multi-disciplinary/multi-agency working
Skills
* Evidence of effective interpersonal skills
* Ability to work independently in a community setting
