We are seeking to appoint dedicated, highly skilled and experienced Community Support Practitioners to join our Housing Related Support Service. We are looking for these dynamic individuals to join our team on a full-time basis, and you will be key members of the Hull HRS Team.
Your key responsibilities will include:
To provide a programme of person-centred support to customers referred into the HRS in accordance with contract requirements.
To engage with customers on a weekly basis to support individuals to secure, maintain and achieve independent living in the community.
The co-provision of support plans, risk management plans, crisis intervention and engaging with customers with multiple and complex needs in order to deliver positive outcomes in a service that offers involvement and empowerment for customers.
To provide a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers. Engaging with customers to design services that work for them and utilising resources within the local area to make a real, quality difference to peoples' lives.
Your essential skills and requirements will include:
Educated to NVQ Level 3 or willing to work towards.
A sound understanding of the social housing environment including emerging government policy.
Experience of working in a social care or similar environment.
Knowledge of the issues faced by customers that have gone through the Criminal Justice System.
Knowledge of varying Complex Needs and their impact on the ability for a person to maintain independent living.
Knowledge of welfare benefits and housing benefit entitlement.
Working experience of dealing with complex service users/customers/public.
Excellent organisational and time management skills and the ability to effectively manage a caseload.
