Job summary
We are looking for a highly motivated, caring and organised individual to join our fast-paced reception and administration department to support the efficient day-to-day running of our practice. The role is for a hour post that will be worked over 4 days in the working week (Monday - Friday) from 08:00 - 18:30. Day off work to be confirmed with successful candidate on acceptance of the role.
Main duties of the job
The role of Care Navigator at the Practice encompasses a range of patient facing and admin duties. This includes answering all incoming phone calls to the practice and queries at the front desk, making phone calls and sending text messages/emails/letters, and completing all other admin duties required to provide our patients with the best care possible (see job description for more details).

Job description
Job responsibilities
1. Answering incoming calls from patients and any other third parties ( pharmacies, hospital, nursing homes)
2. Dealing directly with patient and all other visitor queries on the front reception desk
3. Dealing with a wide variety of patient queries appointments, prescriptions, results, referrals and lab tests
4. Booking appointments for patients and considering demand and patient need in order to do this effectively home visits, telephone consultations
5. Making phone calls on behalf of clinicians asking patients to make an appointment
6. Communicating various clinical results to patients
7. Inputting appropriate coding and clinical data onto patient notes diagnoses, hospital admissions
8. Summarising medical records
9. Photocopying, filing, emailing and printing of patient related documents
10. Opening all incoming mail to the practice and distributing to all departments
11. Typing and sending of letters
12. Ordering clinical tests for patients
13. Effectively utilising the various practice clinical systems
14. Effectively utilising Microsoft packages
15. Chaperoning GP when required after training
16. Dispose of all confidential waste according to practice protocol
17. Any other duties as authorized by the Care Navigation Hub Lead / Practice Manager

21. Previous GP surgery experience (but open to other relevant transferable skill set experience as full training will be given on all aspects of role).
